Kansas Health Officials Highlight Concerns Over 7-Hydroxymitragynine

Kansas City, Mo. — The Kansas Department of Health and Environment (KDHE), alongside the U.S. Food and Drug Administration (FDA), recently issued an advisory regarding 7-hydroxymitragynine (7-OH), a compound that naturally occurs in the kratom plant, but only in very small amounts. While kratom and 7-OH are related, they are not the same. Current regulatory discussions focus on concentrated or synthetic forms of 7-OH, which differ significantly from the naturally occurring levels found in the plant.

This announcement reflects an ongoing national discussion about how kratom and its related compounds should be approached from public health, regulatory, and consumer perspectives.

What Is 7-Hydroxymitragynine

7-Hydroxymitragynine is one of several alkaloids found in Mitragyna speciosa, commonly known as kratom, a tree native to Southeast Asia. Naturally, the compound occurs in small amounts in kratom leaves, with mitragynine being the primary alkaloid. According to researchers, 7-OH may act on opioid receptors in the body and can be significantly more potent than mitragynine when isolated or concentrated (National Center for Biotechnology Information).

In its natural context, kratom has been used traditionally in countries like Thailand and Indonesia for centuries, primarily by chewing fresh leaves or brewing dried leaves as tea. The presence of 7-OH in those forms is considered relatively minimal compared to manufactured products that emphasize concentrated extracts.

Kansas and FDA Warnings

KDHE’s advisory highlighted that some products now available in smoke shops, convenience stores, and online may contain concentrated or synthetic 7-OH. The department reported that adverse effects have included anxiety, gastrointestinal distress, insomnia, seizures, and withdrawal-like symptoms

The FDA has also previously cautioned consumers, noting that kratom is not approved as a dietary supplement or drug in the United States. The agency emphasizes that research on kratom’s compounds, including 7-OH, is ongoing, and more data is needed before drawing definitive conclusions about safety or therapeutic value.

Industry and Consumer Reactions

Industry stakeholders have expressed varying perspectives on 7-OH advisories. Some companies have stated that concentrated forms of 7-OH are distinct from natural kratom products and should be evaluated separately. They argue that conflating natural kratom use with synthetic isolates can create confusion for consumers.

Consumer advocates, meanwhile, emphasize the importance of clear labeling and third-party testing to help individuals make informed decisions. The American Kratom Association (AKA) , a nonprofit advocacy group, has long supported standards that promote responsible manufacturing practices. The group maintains that regulated kratom products can be sold safely under appropriate guidelines.

Scientific Context and Ongoing Research

The scientific community continues to investigate how kratom alkaloids function in the human body. Animal studies suggest that 7-OH has opioid-like activity, but human clinical data remain limited. Researchers stress the need for controlled studies to understand better potential therapeutic applications, as well as risks associated with misuse or over-concentration. For example, a study published in Drug and Alcohol Dependence noted that while kratom alkaloids interact with opioid receptors, they may also display different pharmacological profiles compared to classical opioids — fueling debates over whether kratom and its compounds should be regulated similarly to conventional opioids or treated under a unique framework.

Kansas is not alone in addressing kratom-related concerns. Several states have introduced or passed Kratom Consumer Protection Acts (KCPAs), which establish labeling, testing, and age-restriction requirements rather than outright bans. Colorado, for instance, enacted such legislation to strike a balance between access and consumer safeguards (Colorado General Assembly).

At the federal level, the FDA has recommended that the Drug Enforcement Administrationevaluate scheduling certain kratom alkaloids, including 7-OH. However, decisions remain pending, and public comment periods have drawn thousands of responses from researchers, consumers, and advocacy groups.

Balancing Risks and Perspectives

Neutral reporting requires distinguishing between kratom as a plant and isolated or synthetic derivatives like concentrated 7-OH. While regulators caution about the possible harms of over-concentrated products, many users report different experiences with traditional kratom preparations.

Public health experts suggest that the conversation should focus on evidence-based regulation—ensuring products are accurately labeled, free from contaminants, and marketed responsibly. Such measures, they argue, can help consumers make informed choices while allowing researchers to continue studying both risks and potential benefits.

For Kansas residents and the broader U.S. public, the current advisory is a reminder of the importance of staying informed about emerging products.
